Аннотация

The aim of the research work is to improve the results of surgical treatment of patients with edematous-ascitic syndrome and hepatocellular insufficiency by developing original methods of organ-protection and extracorporeal detoxification. Research objectives: were 905 patients with varying degrees of severity of hepatic failure are examined and treated in RSSPMCS named after acad. V. Vahidov for the period from 1976 to 2015. The scientific novelty of the research consists of follow ing: a method of ascitolytic absorption was created in patients with liver cirrhosis complicated by ascitic syndrome; the cause-effect relationships between various indicators of porto-hepatic hemodynamics and the development of ascitic syndrome in patients with CP after various variants of portosystemic shunting were studied; actuarial survival curves after portosystemic shunting in patients with CP, decompensated for edematous-ascitic syndrome, with the definition of prognostic value of factors of functional decompensation of hepatocytes; in conditions "in vitro" the effectiveness of various methods of organ protection and extracorporeal detoxification of the organism in acute hepatocellular insufficiency and edematous-ascitic syndrome; it has been proved that ascitic concentrate can be considered as a transfusion protein solution with a high content of total protein and its albumin fraction intended for substitution therapy; the morphology of peripheral blood in the aspect of predicting the development of acute renal failure and its outcomes in the immediate postoperative period in patients with diseases of the heart, liver and extrahepatic biliary tract; the value of express monitoring of erythrocyte forms for non-specific morphological determination of the severity degree of hemic and circulatory hypoxia processes caused by hemocirculatory micro-occlusion with violation of intrahepatic metabolic processes is determined
