Оптимизация диагностики и лечения острого деструктивного панкреатита

Аннотация

The aims of research is the improvement of results of complex therapy of acute destructive pancreatitis by development optimal systematic multidisciplinary tactics of patients’ management. The scientific novelty of the research is as follows: This study is one the first studies in which the prevalence, rate and features of clinical course of AP in different regions of the Republic of Uzbekistan are determined on the basis of medical-social statistical analysis; for the first time the experience of AP treatment is summarized and it proved that established universal system of emergency medicine implements the principle "... each citizen of Uzbekistan regardless of where he/she lives, how his/her material opportunities are must get the necessary standard of emergency medicine” with maximal effect and results; the modified and adapted to the conditions of Republic research center of emergency medicine method of prediction of pathological-morphological forms and severity of clinical course of AP is offered based on efficacy analysis of clinical, laboratory, radiological and instrumental methods in complex diagnosis and assessment of AP severity; qualitative and quantitative criteria of assessment of risk of destructive AP development, severity degrees, complications and mortality are determined based on correlation analysis of data of different examination methods; The efficacy is evaluated and indications to different methods of AP treatment are developed, the reasonability to early activation of surgical treatment depending on the etiology and disease stage, severity and the extend of destructive complications is determined; rational algorithms of surgical tactics are developed in the light of the analysis of efficacy of various treatment-tactic approaches and methods in complex treatment of AP, and common protocol of intensive therapy in SAP is standardized; the necessity of anticipatory endoscopic examination and treatment administered even in the absence of appropriate clinical, instrumental and laboratory data is proved by critical analysis of treatment results of AP with TS MDP.
