Оптимизация диагностики и лечения инфекционных осложнений у больных острыми лейкозами

Subject of research: 272 patients with acute leukemias in febrile ncu-troncnia. Purpose of work: to research of infections agents resistance structure in acute leukemia patients with febrile neutropenia and implementation of antibacterial therapy for the treatment chart. Methods of research: Clinical, microbiological, statistics. The results obtained and their novelty: Etiological structure of bacterial in-fectional agents in acute leukemia patients in Uzbekistan has been determined first in representative material. The basic agent of bacteriemia Staphylococcus cpidcr-midis (45,8%) was presented, the main agent of nasopharyngeal infection Streptococcus hacmolyticus (36%) was presented in this research. Maximal efficiency of cephalosporin group - ccphapcrazone/sulbactam and ccphcpime in gram-positive and aminoglycoside group - amikacyn in gram-negative infections was proved in studies of bacterial sensitivity to antibiotic treatment. Bacterial sepsis development is registered in reliable more frequently in acute nonlymphoblastic leukosis patients as compared with lymphoblastic leucosis patients. Practical value: Studies, performing here, permit of work out optimal scheme of antibacterial therapy in acute leucosis patients, so - to perform more delicate agents, to decrease fatal outcomes and finally - to reduce hospital expenses. For the reason of these results changes in antibacterial therapy chart were performed. The combination of ccphapcrazone/sulbactam or cephcpimc and amikacyn was prescribed for febrile neutropenic patients in the first step of therapy and combination of ccphapcrazone, ccphapcrazone/sulbactam and amikacyn was prescribed in nasopharyngeal infections. Degree of embed and economic effectively: the results of this study were introduced into the clinic practice Institute of Hematology blood transfusion, materials used in the learning process in the Hematology Department of the Medical Academy of advanced studies.
