Совершенствование лечебно-диагностических аспектов повреждений вертлужной впадины

Аннотация

Subjects of research: pelvis, acetabulum, hip joint. Aim of research: Improving treatment results of patients with IA using modem technology diagnosis, elaborating apparatus of external fixation and methods of using them, with implantation of the last ones to clinical practicum. Method of research: roentgenography, computer tomography, using of spoke-rod apparatus (SRA) The results obtained and their novelty: New treatment-diagnostic management of patients with IA is elaborated. New model of SRA of external fixation and its using technique in different variants of IA, which allows reposition of bone fragments of IA, eliminating central dislocation of head of thigh bone and creating condition for stable fixation of pelvis arc offered. Ability of early activisation of patients in posttraumatic period while using methods of extrafoeal osteosynthesis of IA with SRA external fixation arc founded. Among 62 patients in the further periods the best results arc gained in 26 patients (41.9%), good - in 21 patients (33.8%), satisfactory - in 10 patients (16.1 %), unsatisfactory - in 5 patients (8.2 %). Practical value: Using elaborated diagnostic complex allows detecting character of IA and optimal treatment tactics more reliably. Offered “pelvisthigh” SRA allows expanding diapason of indications to extrafoeal osteosynthesis of acetabulum fractures, decreasing level of operation’s trauma, realize abilities early activisation and rehabilitation of patients at the treatment stages, shortening patients staying period in hospital. Implantation of SRA external fixation and its using technique to clinical practicum will increase quality of specialized aid to those contingent patients. Degree of embed and economic effectivity: Developed treatmentdiagnostic algorithm SRA external fixation at treatment acetabulum fractures arc used in RRCEM and its filials, in the department of compressive-distraction osteosynthesis in SRITO Minister of Health of Rcpublick of Uzbekistan.
