Оптимизация нейрохирургических подходов к лечению малых геморрагических инсульт-гематом

Аннотация

Subjects of research: patients with small stroke-hematomas. Purpose of work: Improving results of treatment of patients with small strokehematomas by using minimally invasive surgical methods. Methods of research: clinic-neurologic evaluation of patients’ condition, computcrcd tomography of brain, selective and computcrcd tomography angiography of cerebral vessels The results obtained and their novelty: Techniques of small strokehematoma’s removal using neurocndoscopy was advanced, which allow improving visualization of relationship of blood clots and anatomic structures of brain, decreasing of possibility of intraoperative injury of brain. There is proved that using ncurocndoscopic puncture removal of deep localized small stroke-hematomas in combination with local fibrinolysis in postoperative period improves treatment results significantly. Practical value: Developed neurocndoscopy assisted technique of small stroke-hematomas removal increases efficiency of surgical intervention at the expense of minimal traumatism and reduction of surgery’s duration without prejudice to hematoma removal radicalism. Implying to clinical practice of offered tactics of defining indications for surgical treatment and using minimally invasive removal of hematomas using neurocndoscopy and local fibrinolysis allows to improving quality of special neurosurgical aid to patients with small stroke-hematomas significantly. Degree of embed and economic effectivity: Developed algorithm of choosing of treatment method of small stroke-hematomas, ncurocndoscopic puncture and neurocndoscopy assisted removal of hematomas arc embed in clinical practice of Republican research center of emergency medicine.
