Влияние меторексата на состояние кардиоваскулярной системы у больных ревматоидным артритом молодого возраста

Subject of inquiry: 102 rheumatoid arthritis patients (RA), from them 68 take methotrexate (MTX). Control group contained 20 healthy volunteers of comparable sex and age. Aim of inquiry: To study the influence of MTX on measures of central hemodynamya, on structural and functional attribute of magisterial arteries, on lipid specter of blood and development recommendations by early diagnostic measures of heart and vessels defeats in patients with rheumatoid arthritis. Methods of investigation: clinical investigation of the patients, clinical and biochemical analysis, acute phase findings, coagulogramm, lipid specter of blood, rheumatoid factor. Echocardiography (EchoKG) with assessment of common structural-geometric diastolic and systolic parameters of left regions was performed in all cases. Endotelial function was assessed with ultrasonic method with determined thickness of intima-media complex (IMC). The results achieved and their novelty: clinic - functional damage of heart and vessels at RA, observations of the structural-functional significants of cardiovasculary system in correlation with dislipidemia in patients who treated with MTX have been carried out. Comparative effectiveness of fenofibrat for treating the patients with RA who treated with MTX and had dislipidemia has been proved. Practical value: There suggested to include into the complex of diagnostics the methods of easier performance to early detection of cardiovasculary system changes at RA. The obtained findings allow optimizing the treatment for the patients with RA using the developed methods of investigations in the treatment of heart and vessels damage in prophylactic institutions. Degree of embed and economic effectively: Study results have been introduced into the practice of Republican Rhcumatologic Center, in rhcumatologic and cardiologic departments and in clinics of TMA. The obtained results have been included in the academic process of the department of internal diseases of TMA. Sphere of usage: therapy and rheumatology.
