Выбор метода хирургического лечения при местно-распространенном раке желудка, осложненного кровотечением

Аннотация

Subjects of the inquiry: 240 locally advanced gastric cancer, complicated with bleeding patients. Aim of the inquiry: to improve the treatment results of locally advanced gastric cancer complicated with bleeding with expanding indications for surgical treatment. Method of inquiry: clinical, roentgenological, computed tomography, ultrasound, gastroscopy, lab tests, Algover’s shock index. The results achieved and their novelty: locally advanced gastric cancer, complicated with bleeding in Ш-IV-stages is not the contraindication to perform the expandcd-combined operations. Enodscopic locally use of 70% etanol and 0.5% lagodcn have high therapeutic efficacy in gastric cancer with bleeding by Forrest IAB. Suggested method contributed to stop of bleeding in 86.4% patients. It was proved, that bleeding and anemia degrees did not influence to the immediate results of the surgical treatment in using worked on pre- and postoperative complex actions. Practical value: there were worked on complex treatment actions in prcopcrative period, which contribute to the decreasing of postoperative complications and lethality, allows expanding indications for surgical treatment and at the end lead to the improving of treatment results of locally advanced gastric cancer, complicated with bleeding. With aim of coordination the practical activities of oncologists and surgeons in emergency medicine there were worked on the concrete surgical tactics in locally advanced gastric cancer, complicated with bleeding, with provision for bleeding, anemia degrees and the period of performed operation. Worked on informative parameters of mathematic prognosing of surgical treatment results in locally advanced gastric cancer, complicated with bleeding arc leading prognostic factors of determination risk degree and development of postoperative complications. Degree of embed and economical effectivity: given development is introduced in the abdominal oncology department of the National Research Center of Oncology and Samarkand branch of the Emergency center.
