Совершенствование диагностики геморрагических диатезов в детско-подростковой популяции в условиях поликлиники

Аннотация

Subjects of research: Children aged from 3 months to 10 years and teenagers from 11 to 18 years with syndrome of hemorrhage of microcirculatory and mixed types. Purpose of work: To develop effective screening program for early diagnostics of hemorrhagic diathesis in children and teenagers in ambulatory conditions. Methods of research: clinical, hematological, coagulative, biochemical, questionnaire design, instrumental, statistical. The results obtained and their novelty: For the first time for primary link of public health an effective screening program for early diagnostics of hemorrhagic diathesis in children and teenagers under ambulatory conditions has been developed and offered; the structure and age specific features of different hemorrhagic forms in studied population have been defined; ambulatory criteria of selection groups with risk of hemorrhage for study their haemostatic system were formed; risk factors in provocation of hemorrhagic syndrome in hereditary thrombocytopathies have been defined; an algorithm and ambulatory criteria in diagnostics of thrombocytopathies and infrequent forms of hemorrhagic diathesis have been developed. Effectivity of models used, algorithms and test for magnesium in diagnostics of the basic forms of thrombocytopathies has been proved. Practical value: It is determined by development and implementation into the clinical practice of regional out-patient' clinics, that is on the level maximally approached to the patient, effective screening-program for early diagnostics of hemorrhagic diathesis in children and teenagers, that increases efficacy of clinical examination of such patients and unload specialized hematological hospitals significantly.
